If you wish to make a complaint or Human Rights Act claim to the Tribunal, you will need to complete the relevant form and send it to us.

Downloadable forms and an information leaflet are available below:

Complaint Form T2
Information Leaflet

Hard copies of the leaflet and forms can be obtained by writing to :

The Investigatory Powers Tribunal
PO Box 33220
London
SW1H 9ZQ

Or by telephoning 0207 035 3711

In order for the Tribunal to investigate a complaint or claim, you will need to supply:

your name
your address
your date of birth
the name of the organisation you are complaining about
telephone numbers, all postal and e-mail addresses (for allegations of interception of telecommunications)
various details of the conduct you are complaining about

During its inquiries, the Tribunal can only disclose your name, address, and date of birth to the organisation you are complaining about. It needs to disclose this information to enable record searches to be made to see if any information is held on a complainant.

The Tribunal requires your permission to disclose any further details regarding your complaint. You can give this permission by ticking the box on the form. Although you do not have to give this permission, the Tribunal may not be able to conduct as thorough an investigation if you do not consent to these details being disclosed.

Please make sure that you complete all relevant personal details, sign and date the form, and check that the conduct you are complaining about relates to one (or more) of the things that the Tribunal can investigate.
